HOW DO I?

Register as a new patient?
If you wish to join our practice, please ask a receptionist for a registration form for each each person that wishes to register. If you have your medical card available please attach it to the form.

You may fill in the form straight away or take it home and return with it at a more convenient time.

The form will ask you for personal details such as your name, address, date of birth, telephone number, previous address and doctor. This information helps the Health Authority to quickly trace your medical records and send them to your new doctor.

Once you have completed and signed the form you will be automatically placed on our list.

Shortly after this, the Health Authority will send you a new medical card with your doctors name on for you to keep.

On the back  of your registration form you are given an option to go on a central register as a donor, if you wish to do so.

Ask a doctor to complete a form for me?
The doctor is able to sign and complete forms and claims on your behalf, however, these often carry a charge. Please ask the receptionist if you wish to know the amount. Many of these charges can be claimed back from your employers or insurance companies.

Examples of forms are:

Private sick notes
Holiday cancellation
Private letters
Insurance claims
Passport photographs

Some forms carry no charge, eg: a drugs history form before going into hospital.

Please leave these forms with the receptionist who will pass them on to the doctor. The doctor has a heavy workload and will not be able to treat such forms as priority over ill patients. You therefore, need to leave a week before your form is available for collection.

Renew my sick note?
1. The day before your sick note runs out, contact the surgery (our 'phones are less busy after the morning surgeries).

2. We will need the following information:

  • Your full name and address
  • The date you want your sick note to start from
  • Your medical condition, which is on your current sick note
  • The duration for which you need it eg: one week

3. If you need a signing off note, you can request this by ringing or calling in, giving your name and address and the date you wish to return to work.

4. Your sick note will be ready within 48 hours.

Order a repeat prescription?
You can only receive a repeat prescription if you have a computerised repeat prescription slip. 

When you have ticked the items you need to..

Bring it to the surgery and pop it into the box at the reception counter, post it with a stamped addressed envelope.

Your prescriptions will take 48 hours to process.

All repeat prescriptions are regularly reviewed so after a period of time the doctor may ask to see you.
